Lutheran Senior LIFE Launches All Inclusive Care

Jersey City, NJ – Thursday, September 30th – Lutheran Senior LIFE at Jersey City has opened its federally recognized Program of All-Inclusive Care for the Elderly, known as PACE. This widely accepted program throughout the United States enhances the quality of life for seniors by managing all of their healthcare needs enabling them to continue to live safely in their community.  The 14,000 square foot facility, located at 377 Jersey Avenue in Suite 310, adjacent to Jersey City Medical Center, provides participants with central access to a team of physicians, nurses, social workers, therapists, and support staff to help keep them healthy, active, and as independent as possible living in their own home environment.

The Lutheran Senior LIFE Center offers primary and specialty medical care; access to medications, supplies, and medical equipment; after-hours care by a physician; adult day services; in-home support services; transportation to and from the facility; therapy; dietary counseling; support services, and much more. The Lutheran Senior LIFE team of healthcare providers offers an interdisciplinary team approach to managing the medical, social, and personal needs of seniors enrolled in the program.

“Participants in the program receive home health care services and adult day services based on their individualized care plan determined by the interdisciplinary team,” explains Peg Bucci, Vice President of Housing and Community Services at Lutheran Social Ministries of New Jersey.  “The Lutheran Senior LIFE program has all of the components needed to keep seniors in their own
home, while properly addressing their healthcare needs.  It also provides peace of mind to families who need help caring for their elderly relative.”

Transportation to the Lutheran Senior LIFE Center is provided as well, assuring participants quick and easy access to comprehensive on-site health care services, a nutritious hot breakfast, lunch, snacks, and a variety of social activities.

To enroll in the Lutheran Senior LIFE program, seniors must be 55 years or older, and able to live safely at home at the time of enrollment, living in Bayonne, Hoboken, Jersey City, North Bergen, Union City, Weehawken, Guttenberg, or Secaucus. Additionally, an individual must be certified to need New Jersey nursing home level of care and want to stay at home.
